Unlock the program 2020-07-18 Unlock the program   A) After the completion of the operation, the operator shall notify the designated guardian to prepare to unlock the lock, and the designated guardian shall notify all the lockers to confirm on the spot whether the lock can be unlocked; B) After confirming that the lock can be unlocked, the designated monitoring personnel shall apply to the personnel on duty for unlocking, and the personnel on duty shall confirm and inform all personnel in the equipment area to open and approve unlocking; C) The guardian shall supervise and inspect the process of unlocking by the lockmaker and operation by the operator in accordance with the requirements of "Ii. Unlock the lock and inspect the isolation process" in the Energy Isolation Sheet; D) Restore the isolation equipment (such as switch and valve) to its original operating condition; E) The guardian shall retrieve the lock, key, locking appliance and label, and notify the person in charge of the equipment area that the equipment can operate; F) The custodial personnel shall hand over the personal locks, keys, locking appliances and labels to the personnel on duty or the operation supervisor; G) If the key is lost by the lockmaker, the guardian shall apply to the person in charge of the equipment area for the use of a spare key. Emergency unlock procedure (including emergency unlock during operation and long term lock during production)   The person in charge of the equipment area decides whether to activate the emergency lockpicking procedure. When the person in charge of the equipment area is not present, the supervisor has the right to decide to start the emergency lockup procedure.   After deciding to start the emergency unlock procedure, the guardian informs all personnel in the unlock area that they are about to unlock;   Before opening the lock, the person in charge of the equipment area or the guardian of the operation should contact the installation personnel to determine the status of the equipment. If the installer of the personal lock cannot be found, the equipment can be checked to determine whether the maintenance work has been completed.   If safety is confirmed, the person in charge of the site or the monitoring personnel shall immediately use the spare key to remove the lock and label;   Restore the isolation equipment (e.g. switch, valve) to its original operating condition;   Inform the personnel in charge of the equipment area that the equipment can be operated;   Notify the lock installer as soon as possible that the lock has been opened.    
Develop an energy isolation plan 2020-07-16 Develop an energy isolation plan The EIP-Energy Isolation Plan shall, in accordance with the contents of the operation, JSA analysis results and other process safety information documents of the operation area, set the first and second Isolation points, and attach the Isolation diagrams. The approval of the first and second Isolation points and the approval of the change management shall also be included in the Energy Isolation Plan. The first isolation point is to close or open the valves on the process line in the work area according to the measures formulated by JSA. For processes or equipment with high risk, secondary isolation points should be set, that is, the blind plate should be installed after the first isolation. The isolation of the battery system cannot remove the energy source, and should be electrically treated. In the case of isolation, appropriate precautions should be taken. After the completion of process isolation, it is necessary to drain and replace equipment, pipeline residual pressure and materials. The effectiveness of the isolation shall be verified before the start of the maintenance work, such as the inching device switch and voltage measurement to confirm the isolation of electrical energy; Confirm the status of the valve, no material discharge from the relief valve, no pressure holding in the system, etc. Maintenance shall not commence until the isolation requirements have been met and if there is a risk that stored energy will re-accumulate to an unacceptable risk level, the isolation shall be continued until the end of the maintenance or until the possibility of such accumulation no longer exists.  
Lockout tagout LOTO 2020-07-14 Lockout tagout LOTO In the process of energy isolation, Lockout Tagout is an effective means to ensure energy isolation. However, Lockout Tagout can only prevent inadvertent operation by personnel and cannot protect deliberate actions. Therefore, the locking tag is considered a serious inviolable measure in the process of energy isolation. The authorizer of the unit to which the energy isolation equipment belongs and the personnel affected by the inspection and maintenance operation shall Lockout tagout of the energy isolation equipment according to the procedure (see Figure 2). The Lockout Tagout of the unit to which the equipment belongs must be installed first and removed last. If the inspection and maintenance operation lasts for several days, the locking device must be kept at all times during the operation. Subsequent teams must verify the control of hazardous energy sources at each shift. If the Lockout Tagout authorized employee does not remove the isolation device before the end of the work day, he/she shall inform his/her supervisor class leader: if the first Lockout Tagout authorized employee has no equipment maintenance work before returning, he/she shall inform the supervisor class leader of the next shift; If the work is handed over to another authorized employee, the employee at Lockout Tagout at the isolation point must be handed over at the installation site to another authorized employee who takes over his or her job. The authorized staff on duty may install their own locks/badges, or the key shall be handed over to the authorized staff on duty. Release quarantine and site recovery After on-site inspection and confirmation by the person in charge of the shift, the inspection and maintenance permit shall be closed according to the operation permit closure process. The on-duty authorizer and the affected inspection and maintenance personnel shall unlock and remove isolation one by one according to the Lockout Tagout and unlocking process to restore the equipment and facilities to the state required by production operation. Before lifting the isolation, it shall pay attention to the risk control of the isolation lifting operation, handle the operation permit as required, implement safety measures and emergency measures to control the operation risk. During the execution of energy isolation, each step needs to be coordinated with the corresponding billing, inspection and recording ledger to ensure the accuracy and reliability of the whole process and finally realize the closed-loop management of the process. The certificates and records of energy isolation shall be interrelated with the certificates of inspection and maintenance operation permits. The premise of LOTO implementation is to develop relevant LOTO procedures, including lock, lock storage (lock, lock station establishment), lock, lock acquisition process and record books, training and certification of LOTO authorizer, LOTO process and other contents. When field conditions do not permit LOTO, alternative protectors must be written and documented to provide equivalent protection and must be as detailed as the LOTO program. Energy isolation is a systematic method to control and isolate energy sources. The correct implementation of energy isolation can effectively protect the safety of inspection and maintenance workers and equipment and property.
